The Partnership for Success Program is designed to ensure that youth who are at the greatest risk of becoming perpetrators or victims of violence, make a successful transition to adulthood through intensive monitoring; social support and linkages to community-based comprehensive services.  The goal of this program is to ensure that each youth participant leaves the program as a self-supporting, responsible, reliable young adult who is able to contribute to his/her family and society in a positive manner.  ACM; local police, local mental health, and employment services departments;  and local criminal and juvenile justice systems work together to ensure that participants receive appropriate “wrap around” services to increase the likelihood of successful transitions back into society and reintegration with their families.  The Alliance’s success demonstrates that it has developed and employed a successful model deserving to be replicated. 

Prospective clients/participants must meet one or more of the following criteria:

An aftercare agreement with the local agency responsible for youth rehabilitative services of no less than six months;
Placement in the community or at home;
Have one or more adjudications for a crime of violence/crime against a person.

Through this program, partnerships are formed and maintained with all of the actors (public and private) that must work together to provide targeted at risk youth with the support required to make a successful transition to adulthood.   The program requires mandatory daily face-to-face contact with youth and their monitors/advocates.  In addition, each client must participate in weekly and/or monthly face-to-face case review sessions with appropriate program personnel and partners.  For example, in ACM’s Washington, DC Partnership for Success Program, each client meets with the Family Director, Program Director, Case Manager and Outreach Monitors/Advocates.  At a minimum, each client is required to participate in the following: 

Initial face-to-face intake assessment meeting,
Monthly scheduled case review evaluation sessions,
Monthly mandatory family counseling sessions, and
Special events, activities and recognition ceremonies.
